Better you should include same name affidavit along with supporting documents where you happen to have different names.
Sonia Pereira there was an option to upload additional / optimal docs, that placeholder should there at end of list.
Muzzammil Ajmal Khan when you apply during PR, there is nowhere you can upload optional docs.
