I received the same when I gave my General Ielts few years back. It is generally when you have major gap between the band of individual section. i.e. I was having 9 in reading and and 6.5 in writing. May be that is the general reason. I got my result after 5-10 days.
Is there a chance that they must have given someone else’s (same name)decision to your sister? ; do they mention location details of test?
Appeal the decision they have attached a link .. if something did happen during the exam explain your version of the situation.
