Now I'd like to apply PGWP. Do I need to include my travel history info and all stamped pages of my passport while applying PGWP? Can anyone tell me how should I proceed further?

My understanding if there is no issue with your travel history like no rejection no over stay than you should mention your travel history during your PGWP, it should be fine.
Sugin Anthony then you may end up landing in misrepresentation, if you mention that travel history in any future applications like PR.
