I recently registered an account on GC Key and am currently on the page "Your document checklist" (screenshot attached) and stuck on three questions I didn't specify that I wanted to apply through the SDS category anywhere. Where will I get to specify that ? Will it appear when I submit all of the requested information on this page? I learned that SDS requires an upfront medical examination. What is the process for submitting that request? Can I go to a Panel Physician in my city on my own or will the system assign one to me? I'm curious as to how that mechanism works. Although, It was asked for a declaration if I have taken a test from a designated agency to access my proficiency in English or french. How do we submit our IELTS result score card Your valuable input would be greatly appreciated.

Bro you don't need to mention SDS or non SDS. Depending upon your answer to specific questions, your file would fall in btw these categories. You need to call the nearest hospital or clinic to make a booking for medical test and no one would assign you.
Point No 2 . No one will assign, you need to get it done as per the designated centres by IRCC and once u answer the question of medical as yes , it will be consider as SDS subject to other conditions of sds
